Chiropractic Membership Plans in Tampa

Month to month. Cancel any time with 30 days’ notice.

Dr. Bobby Koser, DC, chiropractic physician, performing a chiropractic adjustment on a patient's low back in a treatment room at HealthSpine Chiropractic in Carrollwood, Tampa.

A membership is a direct agreement between you and this practice: a simple monthly plan for people who want regular chiropractic care without insurance billing. You see Dr. Bobby Koser, DC, chiropractic physician, at every visit.

The math, before anything else: a chiropractic adjustment alone is $60 at our time-of-service rate. Essentials membership is $55 a month and covers one visit: a progress check with Dr. Koser, your adjustment when your examination shows you need it that day, and one therapy on the same basis, at no additional charge. On services your plan does not include, and that you pay for yourself, your member rate is 25% less than our published rate.

The plans

Four plans, priced by how many visits a month the plan includes. That number is the most you can use in a month at no additional charge. It is not a schedule we recommend, and it does not set your care.

Membership plans
PlanIncluded visits per monthMonthly price
Essentials1 visit$55/month
Wellness2 visits$100/month
Performance4 visits$180/month
Complete10 visits$400/month

Your plan, its monthly price, your visit count and the cancellation terms are all written into your membership agreement. You read it and sign it before your first month is billed.

What each plan covers

A plan is a way to pay for care you are already having. It does not set your care. Your visit schedule comes from your examination, and if what your exam calls for does not match what your plan includes, Dr. Koser will tell you rather than fit you to the plan.

  • Essentials, one visit a month. One visit is covered. If you are in about once a month, this is the plan that covers it, and your member rate applies to anything else you book.
  • Wellness, two visits a month. Two visits are covered. If a typical month for you is two visits, this is the plan that covers both at one monthly price.
  • Performance, four visits a month. Four visits are covered. If your months run heavier than two, this is the plan that covers up to four of them at one monthly price.
  • Complete, ten visits a month. The largest plan, covering up to ten visits in a month. Ten is the ceiling on what the plan covers at no additional charge, not a schedule.

You can switch plans after your first 30 days, and either of us can end a membership with 30 days’ notice in writing, so picking one is not a guess you are stuck with. Nobody has to be on a plan to be seen here. Pay-as-you-go pricing for every visit is published on our pricing page.

Membership fees are paid monthly in advance, by card or bank withdrawal on file, and the membership renews each month until one of us ends it. Member rates apply only under a signed membership agreement and only to care you pay for yourself. Care we bill to a third-party payor, including auto insurance under Florida PIP, health insurance, or workers’ compensation, sits outside your membership and is billed on our standard fee schedule.

What every visit includes

  • A progress check with Dr. Koser
  • Your chiropractic adjustment as clinically indicated
  • One included therapy at no additional charge: laser, muscle work and cupping, therapeutic ultrasound, electrical stimulation, traction, or taping

What every membership includes

  • 25% off our published rates on chiropractic services your plan does not include, for care you pay for yourself
  • One comprehensive re-examination, available once in each 12 months of membership
  • Extra visits beyond your monthly allowance when they are clinically indicated, at your member rate
  • Month to month. Either of us can end it with 30 days’ written notice, by email or by mail

THE PATIENT AND ANY OTHER PERSON RESPONSIBLE FOR PAYMENT HAS A RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR PAYMENT FOR ANY OTHER SERVICE, EXAMINATION, OR TREATMENT THAT IS PERFORMED AS A RESULT OF AND WITHIN 72 HOURS OF RESPONDING TO THE ADVERTISEMENT FOR THE FREE, DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION, OR TREATMENT.

Member rates on everything else

Some services are not part of the monthly visit: dry needling, ultrasound-guided dry needling, acupuncture, and Wellness and Performance visits. Members pay less. Rates are set in the membership agreement. Member rates are for care you pay for yourself. Care we bill to an insurer is billed on our standard fee schedule, and devices such as custom orthotics are priced by type and model, with the price given to you before anything is ordered.

How to start

New here with something that hurts? Start with the Pain Source Exam. Dr. Koser examines you, explains what he found, and treats you the same visit when it is appropriate. Join within twelve months of that exam and there is no enrollment evaluation fee, because he has already examined you. The enrollment visit below is the shorter baseline for people who do not need an examination.

Start a Membership: Enrollment Visit, $75

Your $75 covers a focused baseline evaluation, and your plan starts the same visit.

Already examined here in the past year? There’s no enrollment evaluation fee. You just start your first month, and your member rates begin with it. The twelve months run from your last examination here. If you were a member before and cancelled, coming back is Dr. Koser’s decision and anything still owed is settled first, but there is no rejoin fee and time away does not change what you pay.

Not sure yet, or dealing with a specific problem? Book the Pain Source Exam instead. If you start a membership within twelve months of that exam, there is no enrollment evaluation fee. You just start your first month.

What you’re agreeing to

  • Can I cancel? Yes. Month to month. Either of us can end it at any time, for any reason, with 30 days’ written notice by email or mail. You get back the unused part of the final month you paid for.
  • How am I billed? Monthly in advance, by card or bank withdrawal on file, and it renews each month until one of us ends it.
  • Do visits roll over? No. Each month’s visits are used in that month. If you cancel, the unused part of your final month is refunded.
  • Can I pause a membership? No. There is no hold, freeze, or pause. If you need a break, you cancel and rejoin later with no rejoin fee.
  • What if a payment does not go through? A membership that is not paid on time can be cancelled, and an unpaid balance can be sent to a collection agency.
  • Can I switch plans? Yes, after your first 30 days. Moving up takes effect immediately; moving to a smaller plan begins with the next billing cycle.
  • What if I need extra visits? You can add them when they are clinically indicated. Your rate for extra visits is set in your membership agreement.
  • Is this health insurance? No. It is a direct agreement between you and this practice, and the services your membership includes are not billed to your health insurance.*

*Florida law requires your agreement to say this in writing, and it does: the agreement is not health insurance, we will not file claims against your health insurance for the services it covers, it does not count as minimum essential coverage under the Affordable Care Act, and it is not workers’ compensation insurance and does not replace an employer’s obligations. See section 624.27(4), Florida Statutes.

If you have Medicare: call us before you enroll. We will go through how a membership would work for you, and what your care here would cost, before you sign anything.

Injured in a car accident? That care is not a membership visit and does not use the visits in your plan. Book the Auto Accident Injury Visit, and we bill your auto insurance carrier under Florida PIP.

Frequently asked questions

Can I cancel my membership at any time?

Yes. Memberships are month to month, and either of us can end one at any time, for any reason, with 30 days’ written notice. An email or a mailed letter both count, and you get back the unused part of the final month you paid for. We don’t sell prepaid multi-month care packages.

Do unused visits roll over to the next month?

No. Each month’s visits are used in that month and do not carry into the next one. If your membership ends, you are refunded the unused part of the final month you paid for.

What does a membership visit include?

A progress check with Dr. Koser, your chiropractic adjustment as clinically indicated, and one included therapy at no additional charge when a therapy is clinically indicated for you that day: laser, muscle work and cupping, therapeutic ultrasound, electrical stimulation, traction, or taping. If no therapy is indicated, none is performed, and nothing is added to your fee.

How much does it cost to join?

The enrollment visit is $75, which covers a focused baseline evaluation, and your plan starts that same visit. If we have examined you within the prior twelve months, there is no enrollment evaluation fee and you just start your first month. Membership fees are paid monthly in advance by card or bank withdrawal on file. Member rates apply only to care you pay for yourself. Care we bill to a third-party payor, including auto insurance under Florida PIP, health insurance, or workers' compensation, sits outside your membership and is billed on our standard fee schedule.

Is a membership health insurance?

No. It is a direct agreement between you and this practice, and the services included in your membership are not billed to your health insurance. Florida law requires your agreement to say so in writing, and it does.

Do members pay less for other services?

Yes. Members pay 25% less than our published rate for chiropractic services your plan does not include, including acupuncture, dry needling, and ultrasound-guided dry needling. Member rates apply only under a signed membership agreement and only to care you pay for yourself, and your rates are written into it. Care we bill to an insurer is billed on our standard fee schedule.
