• Do visits roll over? No. Visits are use-it-or-lose-it each month and do not roll over.
• Can I switch plans? Yes, you may switch to a different plan after 30 days. Plan upgrades take effect immediately, while downgrades begin with the next billing cycle.
• Can I cancel anytime? Essentials and Wellness Plans require a minimum 90-day commitment. After that period, you may cancel at any time with written notice. Performance and Intensive Care Plans may be canceled at any time. Please note that no refunds are issued for unused visits.
• What if I need additional visits? Additional visits beyond your monthly allowance can be scheduled at a discounted member rate.
• Is this health insurance? No. This is a Direct Primary Care membership that provides access to chiropractic and wellness services without the need for insurance billing.*

*Florida Statute §624.27(4) requires this disclosure: “This agreement is not health insurance, and the primary care provider will not file any claims against the patient’s health insurance policy or plan for reimbursement of any primary care services covered by the agreement. This agreement does not qualify as minimum essential coverage to satisfy the individual shared responsibility provision of the Patient Protection and Affordable Care Act, 26 U.S.C. s. 5000A. This agreement is not workers’ compensation insurance and does not replace an employer’s obligations under chapter 440.”

Medicare Beneficiaries: You are voluntarily opting out of Medicare by enrolling in this membership. Services will not be billed to or reimbursed by Medicare.
